Contaminated Saline and Eye Infections: Diseases, Warning Signs, and Safe Use

Saline looks harmless. It is clear, contains only salt and water in many formulations, and is often described as “gentle” or “similar to natural tears.” That can create a dangerous assumption: if a liquid is saline, it must be safe to put in the eyes.

It is not that simple.

For a solution to be safely used in or around the eyes, it must be appropriate for that specific purpose and remain sterile until use. A bottle may become contaminated during manufacturing, through improper storage, after repeated opening, or when the tip touches a hand, eyelid, contact lens, sink, or other surface. Homemade saline can also contain microorganisms even when it looks perfectly clean.

Depending on how the solution is used, contaminated saline may cause anything from mild conjunctivitis to microbial keratitis, a corneal ulcer, permanent scarring, or a serious internal eye infection. People who wear contact lenses, have recently had eye surgery, have an injured cornea, or have weakened immune systems face particularly high risks.

Understanding the difference between sterile saline, disinfecting solution, eye drops, and emergency irrigation can help prevent a small mistake from becoming a vision-threatening emergency.

Why Contaminated Saline Can Be Dangerous

The surface of the eye has natural defenses, including blinking, tears, and an intact corneal epithelium. However, those defenses are not unlimited.

Contact lenses can create tiny scratches, trap microorganisms against the cornea, and reduce oxygen reaching the eye. Eye surgery, trauma, dry eye disease, and certain medications may also weaken the protective surface. When contaminated fluid reaches a vulnerable eye, bacteria, fungi, or other microorganisms can attach to damaged tissue and multiply.

Saline itself generally does not kill these organisms. It may rinse away loose debris, but it should not be confused with a disinfectant.

The CDC’s contact lens care guidance specifically states that saline does not disinfect contact lenses. It should only be used for rinsing when it is part of a care system recommended by an eye care professional.

A solution can also be contaminated without looking unusual. The liquid may remain clear, odorless, and free of visible particles. Appearance alone cannot confirm sterility.

How Does Saline Become Contaminated?

Contamination can occur before or after a product reaches the consumer.

Manufacturing failures may include inadequate sterilization, insufficient microbial testing, damaged packaging, or poor control of the water used to make the product. For this reason, even commercially manufactured products occasionally become the subject of recalls.

Contamination after purchase is more common and may happen when someone:

  • Touches the bottle tip with a finger
  • Allows the tip to touch the eye, lashes, or contact lens
  • Leaves the cap loose or off
  • Shares a bottle with another person
  • Uses a single-dose vial more than once
  • Transfers solution into a smaller travel container
  • Mixes fresh solution with old liquid
  • Uses a bottle after its expiration or discard date
  • Stores it in excessive heat or direct sunlight
  • Adds tap or bottled water to the container
  • Makes saline at home
  • Uses nasal or wound-irrigation saline in the eye
  • Keeps a large opened bottle for repeated eye rinsing

“Sterile” describes the product under its specified packaging and conditions of use. It does not mean the liquid will remain sterile indefinitely after the seal is broken.

Eye Diseases Associated With Contaminated Saline

The condition that develops depends on the microorganism, the route of exposure, the health of the eye, and how quickly treatment begins.

Bacterial Conjunctivitis

Conjunctivitis affects the conjunctiva, the thin membrane covering the white part of the eye and the inside of the eyelids. Bacterial infection may cause redness, irritation, swelling, and yellow or green discharge.

Conjunctivitis is generally less dangerous than a corneal infection, but symptoms can overlap. A person should not assume that a red, painful eye is “just pink eye,” especially when contact lenses or potentially contaminated products are involved.

Bacterial Keratitis

Bacterial keratitis is an infection of the cornea—the clear front surface of the eye. It can progress rapidly and may produce an open sore known as a corneal ulcer.

Symptoms may include:

  • Significant eye pain
  • Increasing redness
  • Blurred or reduced vision
  • Sensitivity to light
  • Excessive tearing
  • Discharge
  • Swollen eyelids
  • A white or gray spot on the cornea

Pseudomonas aeruginosa is one of the organisms associated with aggressive corneal infections, particularly among contact lens wearers. Without prompt treatment, bacterial keratitis can lead to corneal scarring, perforation, vision loss, or blindness.

The CDC advises contact lens wearers to remove their lenses and contact an eye doctor immediately if they develop pain, redness, blurred vision, light sensitivity, excessive tearing, or discharge.

Acanthamoeba Keratitis

Acanthamoeba is a microscopic organism commonly found in water and soil. It can cause a rare but extremely painful corneal infection that is difficult to diagnose and treat.

Homemade saline is particularly concerning because the water used to prepare it may contain microorganisms. Boiling, filtering, or using distilled water at home does not create a reliably sterile ophthalmic product.

A historical CDC investigation offers a concrete example. In a study of soft contact lens wearers with Acanthamoeba keratitis, 21 of 27 patients—78%—reported using homemade saline, compared with 14 of 81 control participants. The affected patients were also more likely to disinfect their lenses less frequently than recommended and to wear them while swimming. The findings were published in the CDC’s report on Acanthamoeba keratitis in soft contact lens wearers.

Treatment for Acanthamoeba keratitis can be lengthy. In severe cases, corneal transplantation may be necessary.

Fungal Keratitis

Fungi may enter the eye through contaminated products, plant-related injuries, or damaged tissue. Fungal keratitis can cause pain, redness, discharge, light sensitivity, and gradually worsening vision.

Because fungal infections require different treatment from bacterial infections, using leftover antibiotic drops without a professional examination may delay correct treatment.

Endophthalmitis

Endophthalmitis is a serious infection involving the inside of the eye. It is more commonly associated with eye surgery, injections, penetrating injuries, or severe infections than with routine exposure of an intact eye.

However, contaminated irrigation or other sterile products used during surgery can introduce microorganisms directly into vulnerable tissue. Symptoms may include severe pain, rapidly declining vision, intense redness, swollen eyelids, and sometimes visible pus inside the eye.

Endophthalmitis is an emergency that may require injections of antimicrobial medication into the eye and, in some cases, surgery.

Real Recalls Show That Sterility Matters

Contaminated eye products are not merely a theoretical risk.

In 2018, 60,000 units of Gericare Sterile Eye Wash were recalled because of potential microbial contamination that compromised sterility. The company had not received reports of injuries at the time of the announcement, but the FDA recall notice warned that using the affected product could potentially cause a sight-threatening infection.

In 2023, saline products manufactured by Nurse Assist were recalled because their sterility could not be assured. Some affected surgical kits contained the recalled saline for ocular use. The FDA stated that potential consequences included keratitis, pain, light sensitivity, permanent vision impairment, and blindness. The FDA’s ocular kit recall notice provides further details.

Another major 2023 incident involved contaminated artificial tears rather than saline, but it demonstrates what can happen when a product intended to be sterile is contaminated. The FDA and CDC investigated a multistate outbreak involving an extensively drug-resistant strain of Pseudomonas aeruginosa. In the initial FDA alert, 55 patients in 12 states had infections linked by epidemiological and laboratory evidence to the products. Reported outcomes included hospitalization, permanent vision loss, and a death involving a bloodstream infection. The FDA advised consumers to stop using the affected artificial tears.

These incidents do not mean that approved eye products are generally unsafe. They show why sterility, proper packaging, recall checks, and prompt reporting of unusual symptoms matter.

Saline Is Not Contact Lens Disinfectant

One of the most common and dangerous mistakes is storing reusable contact lenses in plain saline.

Saline may rinse lenses after they have already been cleaned and disinfected through an approved system, but it cannot replace multipurpose disinfecting solution or a properly used hydrogen peroxide system.

The FDA recommends using only sterile saline for rinsing when directed and warns consumers never to use tap water, bottled water, distilled water, or homemade saline on contact lenses. Water exposure has been associated with Acanthamoeba keratitis, which can be resistant to treatment. These recommendations are included in the FDA’s contact lens solution safety guidance.

For reusable lenses:

  1. Wash your hands with soap and water.
  2. Dry them completely with a clean, lint-free towel.
  3. Rub and rinse the lenses as instructed.
  4. Disinfect and store them in fresh approved solution.
  5. Empty the case after every use.
  6. Rub and rinse the case with contact lens solution—not water.
  7. Let the case air-dry upside down with the caps off.
  8. Replace the case at least every three months or as directed.

Never “top off” old solution by adding more liquid. The remaining solution may already contain microorganisms, and mixing it with fresh product reduces disinfection effectiveness.

Should You Make Saline at Home?

Homemade saline may be appropriate for certain non-ophthalmic purposes when recommended by a healthcare professional, but it should not be used to clean, rinse, or store contact lenses.

It also should not be routinely placed into an irritated or injured eye. Household measuring equipment, cooking containers, water, and storage bottles cannot reliably provide the sterility required for ophthalmic use.

Even if the salt concentration is correct, homemade saline may contain bacteria, fungi, amoebae, particles, or chemical residues. Too much or too little salt may also irritate the eye.

Use a sealed, commercially prepared sterile product specifically labeled for ophthalmic use. Nasal spray, wound wash, intravenous saline, and contact lens products are not automatically interchangeable simply because they contain 0.9% sodium chloride.

Safe Practices for Eye Wash and Saline Products

Before using any product in the eye:

  • Confirm that the label specifically permits ophthalmic use
  • Check the expiration date and any discard-after-opening instructions
  • Inspect the package for leaks, broken seals, or damage
  • Wash and dry your hands
  • Avoid touching the dispensing tip
  • Replace the cap immediately
  • Do not share the container
  • Discard single-use vials after one use unless the label explicitly says otherwise
  • Do not transfer the solution to another bottle
  • Stop using any recalled, cloudy, discolored, leaking, or suspicious product

Do not keep an opened bottle merely because a large amount remains. Follow the manufacturer’s discard period, which may be shorter than the printed expiration date.

An Important Exception: Chemical Exposure

Warnings about nonsterile water should never delay emergency flushing after a chemical splash.

If a cleaner, industrial chemical, pesticide, plant sap, or other potentially harmful substance enters the eye, begin rinsing immediately with large amounts of clean, room-temperature or lukewarm water. Do not spend time searching for sterile saline.

Remove contact lenses if this can be done easily and continue irrigation for at least 15 to 20 minutes. Encourage blinking and avoid rubbing the eye. Poison Control emphasizes that every second matters and recommends immediate eye irrigation before seeking additional advice.

In the United States, call Poison Control at 1-800-222-1222 after rinsing begins. Severe pain, vision changes, strong acids or alkalis, or persistent symptoms require urgent medical evaluation.

Emergency chemical decontamination and routine contact lens care are different situations. Tap water is unsuitable for storing contact lenses, but it can be essential for immediately flushing a dangerous chemical from the eye.

What to Do After Using Suspected Contaminated Saline

Stop using the product immediately. If you are wearing contact lenses, remove them and do not put them back in.

Do not rinse the eye repeatedly with another non-ophthalmic product, use someone else’s prescription drops, or apply leftover antibiotics. Avoid steroid eye drops unless they have been prescribed after an examination, because steroids may worsen certain infections or conceal their progression.

Photograph the bottle, lot number, expiration date, and packaging. Keep the product closed and separated from other supplies. Ask the eye doctor whether the solution, contact lenses, or case should be preserved for testing.

Arrange urgent same-day evaluation if you develop pain, light sensitivity, discharge, worsening redness, blurred vision, swelling, or a visible spot on the cornea. Contact lens wearers should be especially cautious because microbial keratitis can worsen rapidly.

Severe pain, sudden vision loss, a penetrating injury, or symptoms following recent eye surgery should be treated as an emergency.

An eye doctor may examine the cornea with a slit lamp and fluorescent dye. When infection is suspected, samples may be collected to identify the organism and choose the correct antimicrobial treatment.

The Bottom Line

Saline is safe for the eyes only when it is sterile, correctly labeled, stored properly, and used for its intended purpose. It is not a disinfectant, and homemade saline should never replace an approved contact lens care system. Once contamination is suspected, continued use is not worth the risk.
